Hospital Extension Programs: Plan and execute programs to extend hospital services to underserved areas or communities, ensuring accessibility to medical care. Documentation & Reporting: Maintain accurate records of patient interactions, hospital services provided, and community outreach activities. Prepare reports for hospital management. Collaboration: Work closely with medical staff, social workers, and administrative teams to identify patient needs and help resolve any issues related to hospital extensions or services
